ABSTRACT

Objective To investigate the mechanism of renal injury induced by changes in flow shear stress (FSS) during renal ischemia/reperfusion (I/R).Methods 1.In vitro,HUVECs were divided into 4 groups:(1) HUVECs were loaded with 12 dyn/cm2 force for 30,45,and 90 min by using plate fluid chamber system.(2) Cells were loaded with FSS for 2 h,and then cultured for 1,3,8 and 12 h respectively;(3) HUVECs were pretreated with 0,1,2,4 and 8 mmol metformin and cultured for 24 h.(4) HUVECs in control group were cultured normally.The expression of p-AMPK/AMPK protein was detected by Western blotting in each group.2.In vivo,16 SD rats with successful establishment of IR model were randomly divided into 4 groups (n =4 in each group):(1) static cold storage (CS) group:isolated kidneys were stored for 4 h;(2) hypothermia machine perfusion (HMP) group:isolated kidneys were continuously perfused with 0 ℃ lactated Ringer's solution for 4 h;(3)metformin treatment group (Met-CS):metformin was intraperitoneally injected 3 days before surgery,and the isolated kidneys were obtained after cold preservation for 4 h;(4)rat kidneys of control group were just subjected to thermal ischemia for 30 min.The injury of renal tissue in each group was observed by TUNEL and HE staining.The expression and distribution of p-AMPK protein in renal tissues were detected by immunohistochemistry.The correlation between FSS loss and AMPK expression in kidney tissue was analyzed.Results The expression of p-AMPK in HUVECs could be up-regulated by FSS,and the expression of p-AMPK protein increased with the prolongation of time.After stopping FSS,the expression of p-AMPK protein in HUVECs gradually decreased with time (P<0.05).Metformin could activate AMPK activity in a concentration-dependent manner (P<0.05).The content of p-AMPK in renal tissue of HMP group was significantly higher than that of CS group (P<0.05).The expression of p-AMPK in renal tissue of HMP group mainly distributed in the renal tubules,and few in glomerular endothelial cells and blood vessels.The apoptosis rate of renal tissue in HMP group was significantly lower than that in CS group (P<0.05).In the HMP group,the damage of the renal tissue was mild,there was no swelling,and the renal tubules were slightly expanded.In the CS group,the renal tissue was severely damaged and the renal tubules were markedly swollen.Conclusion During the course of renal IR in rats,changes in FSS may affect renal tissue damage through the AMPK pathway.

ABSTRACT

Objective To investigate the clinical efficacy of Dahuang Xiaozhi Suppository in treating chronic abacterial prostatitis/chronic pelvic pain syndrome (CABP/CPPS) of moist heat and blood stasis syndrome. Methods Two hundred patients of CABP/CPPS were randomly divided into control group and observation group, 100 cases in each group. 2 groups were disabled anti-infective meidicne and other preparations, and received diet and life intervention. The control group received the treatment of Qianliean Suppository, and the observation group received the treatment of Dahuang Xiaozhi Suppository, one capsule each time, once a day, into the anus depth of about 3–4 cm. Ten days were a treatment course with two day interval between each course of treatment. The treatment lasted for three courses. The clinical efficacy of the two groups was observed and compared. The indexes including the scores of NIH-CPSI, leukocyte count and lecithin corpuscles in prostatic fluid, and urinary flow rate were evaluated. Results The total effective rate was 100% (100/100) in the observation group and 78% (78/100) in the control group, and the observation group was much higher than control group (P<0.05). The scores of NIH-CPSI and leukocyte count in both groups were significantly lower after treatment (P<0.01), and those in observation group weresignificantly lower than those in control group (P<0.01). The cases with lecithin corpuscles++++in both groups were significantly more after treatment (P<0.01), and those in observation group were significantly more than those in control group (P<0.01). The peak flow rate and mean flow rate in both groups were higher after treatment (P<0.01), and it was higher in observation group (P<0.01). Conclusion The efficacy of Dahuang Xiaozhi Suppository in treating CABP/CPPS of moist heat and blood stasis syndrome is remarkable, and it is better than Qianliean Suppository.

ABSTRACT

Objective To evaluate the effect of Lifeport and static cold storage (CS)on preservation of donor kidney. Methods Medline,Embase,Cochrane library,CBMdisc,China National Knowledge Infrastructure,Wanfang database and VIP database were searched by computer,and relevant clinical studies on the effect of Lifeport and CS on preservation of transplant kidney were collected,with the search period from database construction to December 31,2015. According to the inclusion and exclusion criteria,relevant references were selected,quality was evaluated,information was extracted,and risk of bias in the study was evaluated.In addition,Meta analysis was conducted using software Stata 12. Results A total of 16 studies were included with 5 randomized controlled trials (RCT)and 11 retrospective cohort studies (RCS).The results of Meta analysis showed that:compared with Group CS,(1 )for total deceased donor (TDD), incidence of delayed graft function (DGF)decreased in Group Lifeport after transplantation,duration of DGF and average length of stay decreased,and 1﹣year survival rate of kidney increased after operation.The incidence of postoperative acute rejection (AR)and 1﹣year survival rate of patients were similar in two groups.(2)for donor after cardiac death (DCD):postoperative incidence of DGF and average length of stay decreased in Group Lifeport;there was no statistical significance in incidence of primary nonfunction (PNF)and AR,1﹣year survival rate of kidneys and patients between two groups.(3)for expanded criteria donor (ECD):postoperative incidence of PNF decreased and 1﹣year survival rate of kidney increased in Group Lifeport;there was no statistical significance in postoperative incidence of DGF and AR and 1﹣year survival rate of kidney between two groups. Conclusions Application of Lifeport in preservation of donor kidney has certain advantages. However,more high﹣quality studies should be further conducted to verify the study findings due to limited quantity and quality of the study.

ABSTRACT

Objective To discuss ureaplasma urealyticum (Uu) and Mycoplasma hominis (Mh)susceptibility and resistance change trend in Lanzhou recent years .Methods Detected and statistical analyzed the mycoplasma drug sensitivity level of urinary tract infection patients in urinary clinic with Mycoplasma culture sensitivity reagents from 2010 to 2014 .Results Mycoplasma to Josamycin ,Doxycycline and Clarithromycin were more sensitive .In the 12 kinds of antibacterial drugs ,we found the declining trend of Ofloxacin and Spectinomycin drug sensitivity rates ,and upward trend of Minocycline and Josamycin drug sensitivity rates .Con‐clusion Mycoplasma overall drug sensitivity rate has a gradually downward trend ,which indicating a poor drug resistance control . It′s important to strengthen the antibiotic drug surveillance and security management .
